Citing a Medical Report

Use the following format when citing a patient medical record when writing papers or case reports. This citation is based on the 6th ed. APA rules for a technical report. 

How to Cite a Nuclear Medicine Report -- Citation example 

Interpreting Physician (Dictation Date). Exam [italicized]. City: Facility.

Examples:

Morris, L. (June 12, 2011). Left forearm. Orlando: Florida Radiology

      Imaging Princeton.

Wilson, H.[Harriet]. (May 22, 2010). Nuclear medicine bone scan. Orange City, FL:

      Florida Hospital Fish Memorial.

Wilson, H. [Henry]. (June 1, 2011). Right forearm. Orlando: Florida Radiology

      Imaging Oviedo.

 

Remember these APA rules:

  • Invert all authors' names; give surnames and initials for up to and including 7 authors.
  • If the reference list includes different authors with the same surname and first initial, the authors' full first names may be given in brackets.
  • For books and reports the rules state: Capitalize only the first word of the title and of the subtitle, if any, and any proper nouns; italicize the title.
  • Double space references and indent lines after the first.
